Medical Records

A medical record is a personal, legal and confidential document that contains documentation of the health care services received during a visit to the hospital. It is the patient’s right to receive a copy of their medical record once it is complete. Reasons for requesting a copy of a medical record include: continuity of care, disability, legal needs, insurance and personal reasons.

To obtain a copy of your medical record, you must submit a signed authorization form.

Authorization forms are also available in the patient registration office from 8 a.m. – 3 p.m. or the cashier office from 8:30 a.m. – 3:30 p.m., Monday - Friday. The patient registration and the cashier offices are located on the first floor near the main hospital lobby.

Copies of completed records are typically available within 30-60 days of request and can be mailed to the address provided on the consent form or picked up in the patient registration or cashier office at the hospital. A photo ID is required to pick up the medical record in person.

Records being sent directly to a physician’s office are provided at no charge. Medical record requests for attorneys, insurance companies or the patient require a prepaid fee. Fees are based on the number of pages copied. Reports for tests such as MRI, MRA, CT scans and x-rays are automatically sent to the patient’s physician and do not need to be requested by the patient.
