Advance Directives
During the admission process, we are required by law to inquire whether you have made any advanced
directive decisions or arrangements.
An advanced directive (sometimes referred to as a living will or durable power of attorney for healthcare) enable you to express your wishes or to appoint
someone to act on your behalf should you be unable to make health care decisions for yourself. Your directive will be maintained in your medical record at
The Fort Hamilton Hospital. You should also provide a copy to your physician's office. To receive the necessary forms and instructions, call our Pastoral
Care office at 513-867-2584.
Pastoral Services
Clergy from your church are welcome to visit you at any time. Hospital chaplains are available to help
you and your family cope with your hospitalization by helping you through emotional, spiritual and quality of life concerns. If you would like a visit
from our chaplains, please ask your nurse. The hospital chapel is available for you and your family's use. It is located on the first floor near the main
lobby.
Patient Billing
Health care insurance
The Fort Hamilton Hospital recognizes both individual and group health insurance policies, including Medicare, Medicaid and other commercial insurance
plans.
Even though you may have extensive health insurance, you are ultimately responsible for payment of your bill. Any charges not covered by your policy or
any charges your insurance company does not pay within 45 days become your responsibility. You are also responsible for payment if your insurance cannot
be verified before you are discharged.
Arrangements for the payment of your hospital charges are generally made in advance or immediately prior to the admission. For further information, call

The Fort Hamilton Hospital cannot extend credit; however, we do accept VISA and MasterCard.
Notice to our Patients:
Pursuant to Section 3727.12 of the Ohio Revised Code you are entitled, upon request, to a list of the usual and customary charges for a selected number
of x-ray, laboratory, emergency room, operating room, delivery room, physical therapy, occupational therapy and respiratory therapy services.
A copy of these charges can be obtained by calling 513-867-2370 or at the cashier's window in the Patient Accounts Department, located on the first
floor near the lobby.
Patient Rights
Your rights as a patient are important. We recognize that you are an individual with unique needs. Our
goal is to respect your personal dignity by providing compassionate care focused on those individual needs.
We encourage your participation in informed decisions regarding your medical care including the decision to discontinue treatment, to the extent permitted
by law. An ethics committee is available to you and your family as needed by contacting the nurse, the physician, a social worker or Pastoral Care.
We will assist you in exercising your rights and make reasonable attempts to inform you of any responsibilities you may have relative to those rights.
If you would like a copy of your rights as a patient, please ask your nurse to provide one for you.
Television and Telephones
Patients may receive calls from 8 a.m. - 10 p.m. A minimal fee will be charged for the television and
telephones.
